6:30 PM 1. Steve Luczack, Kodiak America, requests a brief and informal discussion about the possibility of rezoning his property on the NW corner of Holladay Blvd and 6200 South to RO or possibly NC (This item is a discussion only – 10 minutes)

2 Ryan Griffiths, Beecher Walker Architects & Melisa Wallentine, Sahara Construction,
request a brief and informal discussion about the possibility of building a 120 bed assisted living center on the corner of 5600 South & Highland Dr. (This item is a discussion only – 10 minutes)

7:00 PM 3. Holladay Village Plat Amendment – 4650 S 2300 East. – HV Zone – Planner: Paul Allred
The applicants are Rockworth Development and the City of Holladay. The City seeks to vacate lots 18-25 of the Peony Gardens Subdivision, approve a new plat for the proposed Holladay Village Plaza Subdivision, terminate a portion of Holladay Blvd. for use as a public street, dedicate Laney Avenue from Holladay Blvd to 2300 East and establish the Holladay Village Plaza for limited public use. (This item is a Public Hearing.)

7:40 PM 4. Health Wave, LLC – 6032 Latour St. – Conditional Use Permit – Home Occupation with Customers, – R-1-10 Zone – Planner: Rick Whiting – Applicant, Karen Thorsen, is a
licensed physical therapist and colon hydrotherapist. She requests to move her practice into her home. (This item is a Public Hearing.)

On Thursday, November 29, 2012 at 3:30 pm a copy of the foregoing notice was posted in conspicuous view in the front foyer of the City of Holladay City Hall, Holladay, Utah. A copy of this notice was faxed to the Salt Lake Tribune and Deseret News, newspapers of general circulation in the City by the Office of the City Recorder. A copy was also faxed or emailed to the Salt Lake County Council, Cottonwood Heights City and Murray City pursuant to Section 10-9A-205 of the Utah Code. The agenda was also posted at city hall, Holladay Library, city internet website at www.cityofholladay.com and state noticing website at http://pmn.utah.gov.
